ELECTRIC HOT WATER HEATER REBATE

By signing this agreement, I consent and agree to participate in Carroll Electric’s Water Heater Rebate Program. I understand that the water heater must be installed and used on my premises as a member of Carroll Electric Cooperative, Inc. I further consent to the installation, servicing and subsequent inspection of the radio control switch on my water heater and I understand the radio control switch must remain in service on my water heater for as long as the Co-op deems appropriate. Should I remove or permit removal or disconnection of the radio control switch, I understand that I will be obligated to repay the Co-op the cash incentive payment and any discount applied to my monthly statement after removal. I understand that the Co-op may from time to time inspect and service the radio control switch and that the radio control switch is property belonging to Carroll Electric. 

In addition to the above items, I understand that I will receive a credit on my monthly statement as long as the radio control switch remains in service and the incentive is offered and authorized by our board of trustees.
